About the examinations of the antenatal care

The first pregnancy check

If possible, it is recommended to have the first pregnancy check performed by the 12th week of pregnancy. This examination is the basis for admission to antenatal care and is the primary time for determining the expected delivery date, including confirmation of intrauterine pregnancy by bimanual examination, vaginal ultrasound, possibly laboratory pregnancy test, colposcopy (if a smear test is due), breast examination, weight and height measurement, and counseling.

Full blood count (hemoglobin, hematocrit, erythrocyte indices, white blood cell count, platelet count), renal function (Se creatinine, urea, eGFR), liver function (GOT, GPT, Se bilirubin), blood sugar, thyroid hormone function (TSH), blood typing and red blood cell antibody screening, Treponema pallidum screening (TPHA+RPR), HbsAg screening, complete urine and sediment (protein, leukocytes, glucose, acetone, UBG, sediment).

Resting ECG

The resting ECG does not require any preparation. During the examination, the patient lies in a supine position and electrodes are placed on the torso and limbs. The test provides a picture of the electrical activity accompanying the heartbeat and its changes. The ECG curve is displayed on a monitor, which is also printed out as curves drawn on a paper strip. You cannot move or speak during the test and must remain still throughout.

Consultation by a medical doctor

The examination can be performed once the laboratory tests and ECG are completed.

General internal organ assessment: family and individual medical history, predisposing and risk factors, and possible complaints. General physical examination, including assessment of the condition of the major organ systems (skin, mucous membranes, lymph nodes, respiratory system, cardiovascular system, abdominal organs, musculoskeletal system, and nervous system), blood pressure, BMI measurement, lifestyle advice. A detailed evaluation of the previously performed resting ECG is given.

2 obstetric checks during each trimester

During the examinations, body weight, pulse, and blood pressure are measured, the position of the fetus, fetal movement, fetal heart function, uterine fundus, uterine contractions are determined, and, if necessary, vaginal exploration, a vaginal examination, a cervix score, if needed, the week of pregnancy is determined, and a urine test strip (protein, glucose-acetone, leukocytes) is examined, if necessary. Checking fetal heart from week 28, teaching pregnant woman how to count fetal movement from week 34. Counseling, discussion of complaints, treatment, if necessary, giving appointment for the next checkup.

1st trimester genetic ultrasound examination elements

Combined test (free beta-HCG, PAPP-A)

The combined test is used for biochemical screening of fetal abnormalities and to assess fetal risk, which can more accurately determine the likelihood of certain fetal abnormalities, including trisomy 21, Down’s disease (85-90%), Edwards disease (trisomy 18) and Patau’s syndrome (trisomy 13).

Full blood count (hemoglobin, hematocrit, erythrocyte indices, white blood cell count, platelet count), complete urine (protein, leukocytes, glucose, acetone, UBG, sediment), stress blood glucose test (fasting and 120 minutes after 75 g glucose consumption).

3rd trimester ultrasound examination elements

Full blood count (hemoglobin, hematocrit, erythrocyte indices, white blood cell count, platelet count), renal function (creatinine, urea, eGFR), liver function (GOT, GPT, Se bilirubin), coagulation parameters (INR and fibrinogen), complete urine and sediment (protein, leukocytes, glucose, acetone, UBG, sediment), blood type and antibody screening, vaginal secretion culture (group B Streptococcus screening).
